Job summary

Holon Health in Arizona seeks an experienced Nurse Practitioner to deliver integrated, whole-person care to individuals with SUD. This remote-friendly role offers flexibility and can be full-time (40 hrs/week) or part-time (20 hrs/week).

You will collaborate with Care Navigators, Recovery Coaches, Case Managers, and community partners via telehealth. Responsibilities include health evaluations, diagnoses, medication management, care coordination, and client education.

Qualifications

• Active and unrestricted NP license in Arizona (multistate licenses are a plus).
• Active DEA license (CDS where applicable).
• Several years of experience in behavioral health, addiction medicine, or psychiatry.
• Board certification in specialty preferred.
• Strong documentation skills and proficiency in EHR platforms.

Responsibilities

• Health evaluations to assess substance use, co-occurring conditions, and overall functioning
• Diagnose conditions and create individualized treatment plans
• Prescribe and manage medications, monitor side effects, and adjust treatment as needed
• Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
• Serve as a clinical consultant on complex or high-need cases
• Educate clients and families on treatment plans, medications, and recovery support strategies
• Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
• Participate in team meetings, professional development, and continuous training
