Job summary

Holon Health in Nevada is seeking an experienced Nurse Practitioner to support individuals with Substance Use Disorder through telehealth-based, integrated care. The role collaborates with Care Navigators, Recovery Coaches, Case Managers, judicial partners, and community organizations to address mental, emotional, and physical health needs.

The position offers remote flexibility and can be full-time or part-time depending on availability.

Qualifications

• Must reside in Nevada
• Active and unrestricted NP license in Nevada
• Active DEA license (CDS where applicable)
• Several years of experience in behavioral health, addiction medicine, or psychiatry
• Board certification in specialty preferred
• Strong documentation skills and proficiency in EHR

Responsibilities

• Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning
• Diagnose health conditions and create individualized treatment plans
• Prescribe and manage medications, monitor for side effects, and adjust treatment as needed
• Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
• Serve as a clinical consultant on complex or high-need cases
• Educate clients and families on treatment plans, medications, and recovery support strategies
• Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
• Participate in team meetings, professional development, and continuous training
