Dreams 2 Reality Recruitment is seeking a Remote Nurse Practitioner to join a collaborative clinical team providing care to medically complex patients through telehealth. The role focuses on value-based care, care coordination, and proactive outreach from a virtual setting.

You will work with providers, health plans, and families to deliver patient-centered care, manage Medicaid/Dual Eligible populations, and support transitions of care while maintaining high-quality outcomes.

Qualifications

• Master's degree in Nursing or related field.

Responsibilities

• Provide clinical assessments, treatment, and ongoing patient management through telehealth.

An innovative healthcare organization is seeking an experienced Remote Nurse Practitioner (NP) to join a collaborative clinical team providing care to medically complex patients in Delaware.

This is a remote, telehealth-focused Nurse Practitioner role that offers the opportunity to deliver patient-centered care from a virtual setting while working as part of an interdisciplinary care team.

The role focuses on value-based care, complex care management, proactive patient outreach, and care coordination , with the goal of helping vulnerable patients remain safely at home while reducing avoidable emergency department visits and hospitalizations.

The ideal candidate has experience in primary care, telehealth, virtual care, care management, value-based care, complex patient care coordination, and/or managed care. About the Remote Nurse Practitioner Role

As a Remote Nurse Practitioner , you will provide clinical care and ongoing management through telehealth and remote care technologies.

Through virtual care, remote patient management, and proactive care coordination, the team works with patients, families, caregivers, primary care providers, specialists, and health plans to:

• Identify and address changes in patient health early
• Provide ongoing clinical support through telehealth
• Coordinate care across providers and healthcare settings
• Prevent avoidable emergency department visits and hospitalizations
• Support patients during hospitalizations and transitions back home
• Develop individualized care plans based on patient needs and goals
• Improve health outcomes and quality of life
• Support patients with complex medical and social needs

Key Responsibilities

As a Remote Nurse Practitioner , responsibilities will include:

• Provide clinical assessments, treatment, and ongoing patient management through telehealth.
• Conduct virtual visits and clinical encounters using remote care technologies.
• Manage medically complex Medicaid and Dual Eligible patient populations.
• Monitor patient conditions and identify changes requiring clinical intervention.

• Coordinate care with patients, families, caregivers, primary care providers, specialists, and health plan partners.
• Provide proactive clinical outreach and follow-up for patients with complex needs.
• Support patients during hospital admissions, discharges, and transitions back to the home.
• Help identify opportunities to prevent avoidable emergency department visits and hospitalizations.
• Participate in advance care planning and goals-of-care discussions with patients and families.
• Participate in shared clinical triage and care-support processes.
• Maintain accurate and timely clinical documentation.
• Support HCC coding, risk adjustment, and quality-gap closure initiatives.
• Contribute to clinical best practices and continuous improvement of care delivery.
• Collaborate with the broader care team to promote high-quality, patient-centered, value-based care.

Required Qualifications

• Master#39;s degree or higher in Nursing or a related field.
• Active Nurse Practitioner license with eligibility to practice in Delaware.
• Family Nurse Practitioner (FNP), Nurse Practitioner (NP), or Adult/Adult-Gerontology Nurse Practitioner certification.
• At least 1 year of experience in one or more of the following:
  • Telemedicine or telehealth care
  • Care management
• Experience caring for medically complex adult and/or geriatric populations.
• Ability to practice independently with appropriate prescriptive authority.
• Strong clinical judgment and decision-making skills.
• Excellent communication and relationship-building skills.
• Comfort providing care through telehealth and remote technologies.

Preferred Qualifications

• 1+ year of managed care experience.
• Experience in value-based care.
• Recent primary care experience.
• Experience managing medically complex patients.
• Experience with telehealth, virtual care, or remote patient management.
• Experience with care coordination and transitions of care.
• Knowledge of HCC coding and risk adjustment.
• Familiarity with quality measures, including STAR and HEDIS.
• Experience with advance care planning and goals-of-care discussions.
• Experience working within an interdisciplinary or collaborative care model.
• Experience caring for Medicaid and/or Dual Eligible populations.
