Essen Health Care is seeking a Remote House Calls Nurse Practitioner to support our expanding in-home medical services program across NJ, OH, PA, NE, and TX. Qualifications

• Active NP license in the required state(s).
• National NP certification (ANCC or AANP).
• Minimum 2+ years of clinical experience as an NP.
• Experience in house calls, home health, geriatrics, primary care, or internal medicine preferred.
• Prior telehealth or remote care experience preferred.
• Strong clinical judgment and comfort managing complex patients remotely.
• Excellent communication and documentation skills in EMR.

Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program.
• Manage chronic conditions and provide care coordination for home-based patients.
• Perform comprehensive patient assessments and care plan adjustments.
• Review patient histories, labs, imaging, and diagnostic results.
• Support medication management and reconciliation.
• Coordinate care with primary care providers, specialists, and care teams.
• Document visits and clinical decisions accurately in the EMR.
• Ensure compliance with clinical protocols, quality standards, and regulatory requirements.
• Participate in interdisciplinary case reviews and quality improvement initiatives.

Job Summary

We are seeking aRemote House Calls Nurse Practitioner in NJ, OH, PA, NE, TX to support our in-home medical services program. This role providesvirtual clinical oversight, patient assessments, care coordination, and provider supportfor patients receiving care in their homes.

This is a fully remote position with a strong focus on quality, efficiency, and patient-centered care.

• Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patientrequiresprimary care services

• Performcare management needs assessments, follow-ups, and care plan adjustments

• Review patient histories, labs, imaging, and diagnostic results

• Supportmedication management and reconciliation

• Coordinate care with primary care providers (if patients already have one), specialists, and care management teams

• Document visits and clinical decisions accurately in the EMR

• Ensure compliance with clinical protocols, quality standards, and regulatory requirements

• Participate in interdisciplinary case reviews and quality improvement initiatives

Qualifications

• ActiveNurse Practitioner (NP) licensein the required state(s)

• National NP certification (ANCC or AANP)

• Minimum2+ years of clinical experienceas a Nurse Practitioner

• Experience inhouse calls, home health, geriatrics, primary care, or internal medicinestrongly preferred

• Priortelehealth or remote care experiencepreferred

• Strong clinical judgment and comfort managing complex patients remotely

• Excellent communication and documentation skills

• Must be licensed in NY and one of the other states listed or willing to obtain

Technical & Regulatory Requirements

• ProficiencywithEMR/EHR systems

• Comfortable using telehealth platforms and remote monitoring tools

• ActiveDEA registration(or willingness to obtain, if applicable)
