Telehealth and remote technologies will enable ongoing management, with collaboration among physicians, an interdisciplinary team and families to improve outcomes.

Qualifications

• Active NP license in Delaware or a bordering state; Delaware licensure support provided.
• FNP or Adult/Adult-Gerontology NP certification.
• 3–5+ years of NP experience.
• Experience with medically complex adult/geriatric populations.
• Telehealth or remote-care experience.
• Knowledge of HCC coding/risk adjustment and quality measures.
• Strong communication and clinical decision-making skills.

Responsibilities
