Job summary

Essen Health Care is seeking a Remote House Calls Nurse Practitioner to provide virtual clinical oversight, patient assessments, and care coordination for in-home services across NJ, OH, PA, NE, and TX. This role emphasizes quality, efficiency, and patient-centered remote care.

The ideal candidate will have an NP license, ANCC or AANP certification, and 2+ years of relevant clinical experience, with strong communication and EMR documentation skills to support care management and telehealth

Qualifications

• Active Nurse Practitioner license in the required state(s).
• National NP certification (ANCC or AANP).
• Minimum 2+ years of clinical experience as a Nurse Practitioner.
• Experience in house calls, home health, geriatrics, primary care, or internal medicine preferred.
• Prior telehealth or remote care experience preferred.
• Strong clinical judgment and comfort managing complex patients remotely.
• Excellent communication and documentation skills.
• Must be licensed in NY and one of the other listed states or willing to obtain.

Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program.
• Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes.
• Perform care management needs assessments, follow-ups, and care plan adjustments.
• Review patient histories, labs, imaging, and diagnostic results.
• Support medication management and reconciliation.
• Coordinate care with primary care providers, specialists, and care management teams.
• Document visits and clinical decisions accurately in the EMR.
• Ensure compliance with clinical protocols, quality standards, and regulatory requirements.
• Participate in interdisciplinary case reviews and quality improvement initiatives.

Job Summary
