This is a remote, telehealth-focused Nurse Practitionerrole that offers the opportunity to deliver patient-centered care from avirtual setting while working as part of an interdisciplinary care team.
The role focuses on value-based care, complex caremanagement, proactive patient outreach, and care coordination, with thegoal of helping vulnerable patients remain safely at home while reducingavoidable emergency department visits and hospitalizations.
The ideal candidate has experience in primary care,telehealth, virtual care, care management, value-based care, complex patientcare coordination, and/or managed care. About the Remote Nurse Practitioner Role
As a Remote Nurse Practitioner, you will provideclinical care and ongoing management through telehealth and remote caretechnologies.
Through virtual care, remote patient management, andproactive care coordination, the team works with patients, families,caregivers, primary care providers, specialists, and health plans to:

Key Responsibilities
As a Remote Nurse Practitioner, responsibilities willinclude:
- Provideclinical assessments, treatment, and ongoing patient management throughtelehealth.
- Conductvirtual visits and clinical encounters using remote care technologies.
- Managemedically complex Medicaid and Dual Eligible patient populations.
- Monitorpatient conditions and identify changes requiring clinical intervention.
- Developand implement individualized care plans in collaboration with physiciansand interdisciplinary team members.
- Coordinatecare with patients, families, caregivers, primary care providers,specialists, and health plan partners.
- Provideproactive clinical outreach and follow-up for patients with complex needs.
- Supportpatients during hospital admissions, discharges, and transitions back tothe home.
- Helpidentify opportunities to prevent avoidable emergency department visitsand hospitalizations.
- Participatein advance care planning and goals-of-care discussions with patients andfamilies.
- Participatein shared clinical triage and care-support processes.
- Maintainaccurate and timely clinical documentation.
- SupportHCC coding, risk adjustment, and quality-gap closure initiatives.
- Contributeto clinical best practices and continuous improvement of care delivery.
- Collaboratewith the broader care team to promote high-quality, patient-centered,value-based care.
Required Qualifications
- Master'sdegree or higher in Nursing or a related field.
- ActiveNurse Practitioner license with eligibility to practice in Delaware.
- Atleast 1 year of experience in one or more of the following:
- Telemedicineor telehealth care
- Caremanagement
- Experiencecaring for medically complex adult and/or geriatric populations.
- Abilityto practice independently with appropriate prescriptive authority.
- Strongclinical judgment and decision-making skills.
- Excellentcommunication and relationship-building skills.
- Comfortproviding care through telehealth and remote technologies.
Preferred Qualifications
- 1year of managed care experience.
- Experiencein value-based care.
- Recentprimary care experience.
- Experiencemanaging medically complex patients.
- Experiencewith telehealth, virtual care, or remote patient management.
- Experiencewith care coordination and transitions of care.
- Knowledgeof HCC coding and risk adjustment.
- Familiaritywith quality measures, including STAR and HEDIS.
- Experiencewith advance care planning and goals-of-care discussions.
- Experienceworking within an interdisciplinary or collaborative care model.
- Experiencecaring for Medicaid and/or Dual Eligible populations.
