Qualifications

• Active and unrestricted RN license in Wisconsin.
• Minimum 2 years of clinical experience in medical-surgical, primary care, or case management.
• Proficiency with EHR systems and Microsoft Office Suite.
• Excellent verbal and written communication skills with diverse patient populations.

Responsibilities

• Serve as a primary point of contact for patients navigating the healthcare system remotely.
• Coordinate patient care with providers, specialists, and other stakeholders.
• Maintain clear, timely communication with patients, caregivers, and professionals via phone, email, and teleconferencing.
• Document patient interactions and care plans in the proprietary app.
• Facilitate referrals to providers, resources, and services as needed.
• Adhere to HIPAA and other privacy laws and stay updated on healthcare trends.
