Job summary

Actalent is seeking a Case Management Nurse in Milwaukee, WI to coordinate comprehensive patient care and advocate for access to services across the continuum of care. The role blends direct client contact with care coordination, education, advocacy, and resource navigation to promote wellness and improve outcomes.

You will manage caseloads, develop care plans, and collaborate with providers and community partners, with a focus on documentation and quality improvement in a remote-friendly setup.

Qualifications

• Active RN license is required.
• Case management or care coordination experience is a plus.
• Strong communication and relationship-building skills are essential.
• Passion for helping others and improving patient outcomes.
• Comfort working with diverse patient needs and psychosocial issues.

Responsibilities

• Manage a caseload of clients, coordinating care and supporting them through their healthcare journey.
• Build personalized care plans and stay connected with clients to ensure they’re getting the support they need.
• Advocate for clients and help remove barriers to care.
• Connect clients with community resources and services.
• Work closely with healthcare providers and community partners.
• Keep documentation accurate and up to date.
