Job summary

Elevance Health is seeking a Nurse Case Manager I (US) to perform care management for members with complex needs. The role can be remote full-time with required in-person training and occasional site visits, centered in Latham, NY.

The schedule runs Monday through Friday, with occasional evening shifts. The position requires an RN license (state-specific, with multi-state licensure where applicable) and a BA/BS in a health-related field with 3 years of clinical care experience.

Qualifications

• RN license in applicable state(s) required.
• BA/BS in health-related field and 3 years of clinical care experience or equivalent background.
• Multi-state licensure is required if providing services in multiple states.

Responsibilities

• Assess and develop care plans for members with complex and chronic conditions.
• Coordinate, monitor, and evaluate care plans across the care continuum.
• Facilitate authorizations/referrals within benefits or through extra-contractual arrangements.
• Interface with Medical Directors and Physician Advisors on care plans.
• Coordinate internal/external resources to meet identified needs.
• Assist in problem solving with providers, claims or service issues.
