Job summary

Elevance Health, Inc. is hiring a Nurse Case Manager I to perform licensure-based care management for members with complex needs. The role involves telephonic or on-site care coordination, including discharge planning and multidisciplinary collaboration.

Qualified candidates hold an RN license (multi-state preferred), a BA/BS in a health-related field, and at least 3 years of clinical experience. Hybrid/virtual work with occasional in-person training is provided.

Responsibilities

• Ensures member access to services appropriate to their health needs.
• Conducts assessments to identify needs and create care plans.

• Coordinates internal and external resources to meet identified needs.
• Monitors and evaluates the care management plan and adjusts as needed.
• Interfaces with Medical Directors and Physicians Advisors on care plans.
