Elevance Health, Inc. is seeking a Nurse Case Manager I to perform care management for members with complex and chronic care needs. The role may be performed virtually with required in-person training sessions and occasional on-site work for discharge planning.

You will assess needs, develop care plans, coordinate resources, and monitor outcomes while collaborating with Medical Directors and Physician Advisors. A valid RN license and multi-state licensure support are required.

Qualifications

• Requires BA/BS in a health related field and minimum of 3 years of clinical experience.
• Current, unrestricted RN license in applicable state(s) required.
• Multi-state licensure is required if providing services in multiple states.

Responsabilités

• Care management within scope of licensure for members with complex and chronic care needs.
• Assess, develop, implement, coordinate, monitor, and evaluate care plans.
• Perform duties telephonically or on-site (e.g., hospital discharge planning).
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate effectiveness of care plans and modify as needed.
• Interface with Medical Directors and Physician Advisors on care plans.
• Negotiate rates of reimbursement as applicable.
• Assist in problem solving with providers, claims or service issues.
