Job summary

Elevance Health is seeking a Nurse Case Manager II to manage complex patient care telephonically or on-site, coordinating plans across the care continuum.

The role requires a RN license, BA/BS in a health field, and 5+ years of clinical experience. Work is virtual full-time with required in-person training; some locations within commuting distance may be considered. Schedule: Monday–Friday, 10:00–6:30 PM EST.

Qualifications

• BA/BS in a health-related field is required.
• Current, unrestricted RN license in applicable state(s).
• Multi-state licensure required if providing services in multiple states.
• Certification as a Case Manager is preferred.

Responsibilities

• Assess, develop, implement, coordinate, monitor, and evaluate care plans for members with complex needs.
• Provide telephonic or on-site care management including discharge planning.
• Facilitate authorizations/referrals within benefits structure or through extra-contractual arrangements.
• Coordinate internal and external resources to meet member needs.
• Monitor and modify care plans based on effectiveness.
• Collaborate with Medical Directors and Physician Advisors on treatment plans.
