Job summary

Cardinal Health is seeking a Nurse Care Management professional to coordinate patient care for individuals in an individualized care program. The role emphasizes structured, patient-centered services, education, and bridging gaps between patients, clinicians, and support systems.

You will manage a caseload of 100-150 members, collaborate with onsite and remote teams, and ensure HIPAA compliance while delivering high-quality care in a remote work setting.

Qualifications

• Registered Nurse with a current, unrestricted California license.
• 5 years’ experience - hospital or clinical, involving patients with complex chronic disease states preferred.
• Care Management experience is strongly preferred.
• Oncology patient experience is strongly preferred.
• Experience with Microsoft Office products.
• Superior communication skills to include verbal and written.
• Must be able to work collaboratively; team focused.
• Excellent organizational skills.
• Bilingual skills a plus.

Responsibilities

• Collaborate with health care staff to develop, implement, monitor and evaluate appropriate clinical care and coordinate activities.
• Manage chronic disease care across oncology patients.
• Document all encounters accurately in the designated system.
• Participate in interdisciplinary case conferences and team huddles for coordinated care.
• Identify patients to be case managed and coordinate their care plans.
• Develop and review clinical pathway trends and share with teams.
• Assist in quality improvement activities by identifying trends and barriers.
• Maintain HIPAA guidelines and ensure compliance.
