Job summary

Cardinal Health is seeking a Nurse Care Manager to coordinate patient care, educate patients, and bridge gaps between patients, healthcare providers and support systems. This remote, full-time role supports Oncology and chronic care management with an emphasis on care transitions and adherence.

Responsibilities include collaborating with clinical staff, documenting in the EHR, maintaining HIPAA compliance, and managing a caseload of 100–150 members. Evening or weekend shifts are not required.

Qualifications

• RN license with current, unrestricted status.
• 5 years’ hospital or clinical experience, preferably in care management.
• Care Management and Oncology patient experience preferred.
• Experience with Microsoft Office products.

Responsibilities

• Collaborate with health care staff to plan and coordinate patient care.
• Monitor and evaluate clinical care for patients.
• Document encounters in the EHR accurately.
• Identify patients for case management with oncology care team.
• Coordinate referrals and scheduling of tests and procedures.
• Ensure HIPAA compliance.
• Manage caseload of 100–150 members.
