Job summary

Cardinal Health is seeking a Nurse, Individualized Care Nurse Care Management to coordinate medical treatments, deliver patient education, and bridge gaps between patients, providers and support systems to promote continuity of care.

The role operates remotely, full-time, with training during 8:00am-5:00pm CST and a Monday-Friday schedule. Anticipated salary range and comprehensive benefits accompany this position.

Qualifications

• Registered Nurse with a current, unrestricted California license.
• 5 years’ hospital or clinical experience with complex chronic disease states preferred.
• Care Management experience strongly preferred.
• Oncology patient experience strongly preferred.
• Experience with Microsoft Office products.
• Superior communication skills (verbal and written).
• Ability to work collaboratively; team focused.
• Excellent organizational skills.
• Bilingual skills a plus.

Responsibilities

• Collaborate with health care staff to develop, implement, monitor and evaluate clinical care and coordinate care management activities.
• Demonstrate knowledge of chronic disease management, including cancer, and ensure regulatory compliance.
• Document encounters accurately in the designated system.
• Participate in interdisciplinary case conferences and team huddles for coordinated care.
• Identify patients for case management and coordinate modification of care with the oncology team.
• Support development and review of clinical pathway trends and share with teams.
• Contribute to quality improvement by identifying trends and barriers to improve outcomes.
• Maintain HIPAA guidelines and ensure patient privacy.
