Job summary

Cardinal Health is seeking a Nurse, Individualized Care Nurse Care Management to coordinate medical treatments, educate patients and bridge gaps between patients and care teams. The role is remote, full-time, with a 40-hour workweek and CST/PST scheduling details.

Candidates will manage 100-150 members, conduct medication reconciliation, and support discharge planning and care transitions. Qualified applicants have a California RN license, 5 years in hospital/clinical settings with chronic

Qualifications

• Registered Nurse with a current, unrestricted California license.
• 5 years’ hospital or clinical experience with complex chronic disease preferred.
• Care Management experience preferred.
• Oncology patient experience preferred.
• Experience with Microsoft Office products.
• Strong communication skills, both verbal and written.
• Ability to collaborate and work in a team.
• Excellent organizational skills.
• Bilingual skills a plus.

Responsibilities

• Collaborate with health care staff to plan and coordinate care.
• Manage a caseload of 100-150 members.
• Medication reconciliation and enrollment outreach.
• Coordinate transitions and discharge planning.
• Ensure HIPAA compliance and accurate documentation.
• Attend meetings and participate in case conferences.
