Job summary

Suvida Healthcareis seeking a Nurse Care Manager to join a multidisciplinary team delivering high-quality care to high-risk patients. You will coordinate chronic disease management, discharge planning, and transitions across care settings to optimize outcomes and resource use.

You will work with physicians, nurses, and care managers to tailor care plans to social determinants of health, monitor progress, and ensure timely documentation in the EMR.

Qualifications

• Bachelor’s degree in Nursing or healthcare related field.
• Experience in chronic care management and transitions of care.
• Strong communication and collaboration across multidisciplinary teams.
• Proficient in documenting care plans and assessments.

Responsibilities

• Collaborate with the multidisciplinary care team to manage high-risk patients.
• Coordinate transitions of care from acute to post-acute settings.
• Develop and implement individualized care plans and follow-up actions.
• Document assessments and care plans in the electronic medical record.
