Qualifications

• RN license (Oregon, Washington and/or Idaho) in good standing
• At least three years of ambulatory care or care coordination experience
• Experience collaborating with multidisciplinary care teams

Responsibilities

• Coordinate care across inpatient, outpatient, and post-acute settings.
• Provide patient education based on condition-specific knowledge and best practices.
• Perform medication and allergy reconciliation with patients and caregivers.
• Communicate with PCPs and care teams for timely follow-up.
• Maintain compliant EHR documentation of patient interactions and care activities.
