Job summary

Cambia Health Solutions is seeking a Remote Care Management Nurse to support members from home across multiple states, including Idaho, Oregon, Utah and Washington. This role focuses on case management, care coordination, and clinical decision-making to deliver quality, cost-effective outcomes.

You will collaborate with physicians and care teams, ensure policy compliance, and maintain thorough documentation while helping members achieve better health.

Qualifications

• Nursing degree (Associate's or Bachelor's) or equivalent
• Active, unrestricted RN license in the U.S.
• Eligibility for licensure in Idaho, Oregon, Utah and Washington

Responsibilities

• Conducts case management activities: assess, plan, coordinate, monitor and evaluate member needs.
• Ensure compliance with medical policy and guidelines using evidence-based criteria.
• Collaborate with physicians, internal/external teams to resolve issues and drive quality improvement.
• Respond to inquiries in a professional manner while protecting confidentiality.
• Provide accurate documentation and meet timelines and performance standards.
• Coordinate resources and prioritize assignments to meet goals and timelines.
