Qualifications

• Active nursing license required.
• 3+ years of experience as a nurse case manager.
• CCM certification or eligibility required.
• In-depth knowledge of healthcare and insurance systems.

Responsibilities

• Serve as the primary point of contact for members.
• Work with members to identify healthcare needs.
• Liaise with TPAs and insurance companies.
• Educate members on their healthcare benefits.
• Advocate for improved healthcare outcomes.
• Collaborate with healthcare professionals.
• Monitor member health status.
• Maintain accurate records of member interactions.
• Provide client-facing reporting.
• Review Health Risk Assessments.
