Job summary

Synergy Healthcare USA, LLC in the Greater Charlotte Area seeks a bi-lingual (Spanish) Nurse Case Manager to advocate for members navigating the healthcare and insurance system. The role supports clients remotely with occasional travel to NC/SC; you will educate members, coordinate care with providers, and liaison with TPAs and insurers.

The ideal candidate has a BSN, 3+ years nursing experience, CCM eligibility or certification, strong analytical skills, and expert EHR/office software

Qualifications

• Active nursing license (RN) preferred.
• Minimum 3 years of experience as a nurse case manager or in a related healthcare field.
• CCM certification or CCM eligible; commit to CCM exam within first year.
• Fluency in Spanish with strong interpersonal skills.
• Ability to work remotely and independently in a fast-paced environment.

Responsibilities

• Serve as primary point of contact for members seeking navigation of the healthcare system.
• Identify member healthcare needs and provide clinical support.
• Liaise with TPAs and insurance companies to resolve claims and billing issues.
• Educate members on benefits and how to utilize them effectively.
• Advocate for members to improve healthcare outcomes, including referrals to specialists.
• Collaborate with physicians and nurses to ensure coordinated care.
• Monitor member health status and progress toward goals.
• Maintain accurate records of interactions and interventions.
• Prepare client-facing reports; occasional travel to client worksites within NC/SC.
