Job summary

Petaluma Health Center is seeking a Nurse Practitioner or Physician Assistant to join the Clinical Support Team. The role focuses on follow-up care, lab and imaging result review, and timely communication between patients and primary care providers.

Based in California with a primarily remote schedule and 1–2 days per week in-person at the Petaluma Clinic. Requires CA licensure, NP/PA program completion, and current board certification.

Qualifications

• Completion of a nationally accredited Nurse Practitioner or Physician Assistant Program.
• Two years’ experience as a licensed NP or PA delivering primary care in family practice or a NP residency.
• Experience with NCQA Patient-Centered Medical Home preferred.
• Experience with Joint Commission Accredited health care organization preferred.

Responsibilities

• Provide patient visits via telehealth to review urgent or timely diagnostic results.
• Provide in person or telehealth urgent care on a regular schedule or as needed.
• Assist in scheduling medical appointments to support patient health.
• Communicate with patients to improve understanding of health care and optimize outcomes.
• Maintain credentials and licensure as required by CA regulations.
