Qualifications

Experience in NCQA Patient-Centered Medical Home preferred.
Bilingual in English and Spanish highly preferred.
Responsibilities
Provide support to teams for timely review of diagnostics.
Conduct telehealth visits to review urgent diagnostic results.
Ensure patients understand their healthcare and assist in scheduling.

Education
