PharmD Live is seeking a Physician Assistant (PA-C) or Nurse Practitioner (NP) to support patients through our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.

This role emphasizes proactive, patient-centered care, chronic disease management, and coordination with a multidisciplinary team in a 100% remote California setting. Strong clinical judgment and HIPAA-compliant workspace are essential.

Qualifications

• Active, unrestricted California PA-C or NP license.
• Current NCCPA, AANP, or ANCC certification.
• 2+ years of clinical experience in primary care, internal medicine, geriatrics, chronic disease management.
• Ability to work independently in a remote, HIPAA-compliant workspace.
• Strong communication and patient-care skills.

Responsibilities

• Conduct clinical and health-risk assessments for patients with chronic and complex conditions.
• Develop, manage, and update individualized care plans.
• Monitor RPM data and identify concerns requiring intervention.
• Provide patient education, coaching, and support on chronic disease management and medications.
• Coordinate care with physicians, pharmacists, nurses, social workers, and other providers.
• Escalate clinical concerns and ensure appropriate follow-up.
• Maintain timely, accurate, and compliant EHR documentation.
• Support CCM/RPM quality, compliance, and patient engagement initiatives.

About the Role

PharmD Live is seeking a Physician Assistant (PA-C) or Nurse Practitioner (NP) to support patients through our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.

Qualifications

• Reliable internet and a private, HIPAA-compliant workspace.

Preferred:

• Experience with CCM, RPM, telehealth, population health, or value-based care.
• Experience managing patients with multiple chronic conditions.
• Familiarity with digital health and remote monitoring platforms.
