Job summary

Local Infusion is seeking a Nurse Practitioner specialized in neurology and memory care for a remote, patient-facing telehealth role. You will assess patients, develop care plans, and prescribe therapies in collaboration with supervising physicians to improve access and time to treatment.

The ideal candidate has 2+ years in neurology/memory care, strong documentation, and excellent communication with patients and families navigating cognitive decline.

Qualifications

• Active, unrestricted NP license in Maryland; geriatrics/adult-gerontology/family practice certification preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.
• Excellent communication skills with patients facing cognitive decline and their caregivers.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
• Partner with neurologists and care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR in line with regulatory requirements.
