Job summary

Local Infusion is seeking a Nurse Practitioner with neurology and memory care expertise for a remote, patient-facing role. You will provide direct telehealth care, develop treatment plans, and collaborate with supervising physicians to improve access and reduce time to treatment.

You will educate patients and families, document thoroughly in the EMR, and work with a multidisciplinary team to ensure continuity of care in a fast-growing startup.

Qualifications

• Active, unrestricted NP license in Maryland.
• Certification in geriatrics, adult-gerontology, or family practice preferred.
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings.
• Strong clinical judgement and clinical documentation skills.
• Comfortable managing a full patient panel independently with physician collaboration.
• Excellent communication skills with patients facing cognitive decline and their caregivers.

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth.
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians including prescribing therapies.
• Partner with our neurologist(s) and broader care team to ensure continuity of care.
• Educate patients and families on diagnoses, treatment plans, and disease progression.
• Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements.
