Qualifications

• Master’s Degree in Social Work, Counseling, or related field is required.
• Virginia Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) required.

Responsibilities

• Assesses the social, psychological, cultural, environmental, and financial situation, and disposition needs for each patient.
• Collaborates with patients, families, healthcare team, and community agencies to develop and implement care plans.
