Job summary

Local Infusion is seeking a Nurse Practitioner specialized in Neurology and Memory Care to join a remote-first care team. The NP will provide telehealth clinical care to patients with neurological conditions, collaborating with physicians to improve access and accelerate treatment.

Applicants must hold an unrestricted Virginia NP license; geriatrics certification is a plus, with 2+ years in neurology or memory care. Strong documentation and patient-centered communication are essential.

Qualifications

• Active, unrestricted NP license in Virginia (required)
• Certification in geriatrics, adult-gerontology, or family practice preferred
• 2+ years of clinical experience in neurology, memory care, and/or dementia care settings
• Strong clinical judgment and documentation skills
• Excellent communication with patients facing cognitive decline and their caregivers

Responsibilities

• Provide direct clinical care to patients with neurological conditions via telehealth
• Conduct patient assessments and develop care plans in collaboration with supervising physicians
• Prescribe therapies as appropriate and manage ongoing treatment
• Partner with neurologists and care team to ensure continuity of care
• Educate patients and families on diagnoses, treatment plans, and disease progression
• Maintain accurate clinical documentation in the EMR in line with regulatory requirements
