Job summary

Sentara Quality Care Network (SQCN) seeks a part-time Social Worker (MSW) for 20 hours per week in Harrisonburg, VA and surrounding areas. Flexible dayshift, 7:30 AM–6:00 PM window; remote work within approved counties; travel may be required.

The role collaborates with Population Health RNs and providers to support value-based care, assist patients in accessing community resources, and advocate for beneficiaries and families.

Qualifications

• 2 years' experience in direct patient care and case management is REQUIRED.

Responsibilities

• Collaborate with Population Health RNs, providers, and staff to engage in management approaches for better health outcomes.
• Advocate for patients, families, and caregivers and liaise between providers, care team members, facilities, and community resources.
• Assist patients in navigating community-based programs and medical care access.
