The position supports a hybrid work model after an initial training period, with standard hours 8:00 AM–5:00 PM, Monday–Friday, and a focus on improving patient care and coordination with community resources.

Qualificações

• Minimum of 2 years of social work experience
• Master's degree in Social Work required
• Licensed Clinical Social Worker (LCSW) required

Responsabilidades

• Participates as part of a multidisciplinary team to develop comprehensive patient plans.
• Provides clinical services and coordinates referrals to community resources.
• Assesses psychosocial needs, including social, cultural, environmental, and financial factors.
• Delivers individual, family and group counseling as needed.
• Provides crisis intervention and bereavement counseling as required.
• Collaborates with team to monitor patient progress and adjust plans.
