Job summary

Sentara Quality Care Network (SQCN) is seeking a Social Worker, MSW for a part-time, flexible dayshift role serving Harrisonburg and nearby areas. The position requires 20 hours weekly with a flexible day schedule, 7:30 AM–6:00 PM EST within the listed locations.

As part of the Population Health Team, you will advocate for patients, coordinate care with RNs and providers, and help connect beneficiaries to community resources to support health outcomes.

Qualifications

• Master’s degree in Social Work REQUIRED.
• Minimum 2 years’ experience in direct patient care and case management REQUIRED.
• Experience with frail or elderly population preferred.
• Knowledge of Medicare, Medicaid and CMS regulations preferred.

Responsibilities

• Advocate for patients, families and care partners across the care continuum.
• Collaborate with Population Health RNs, providers and staff to optimize outcomes.
• Act as liaison between providers, patients, care teams, facilities and community resources.
• Assist patients in navigating community-based programs and access to care.
