Job summary

Sentara Quality Care Network (SQCN) is seeking a Social Worker with a Master of Social Work to fill a part-time, flexible dayshift role across Harrisonburg and surrounding VA communities. You will join the Population Health Team to support patients, families, and care teams, aiming to improve health outcomes and maximize access to community resources.

The role requires a MSW and at least 2 years of direct patient care experience; CMS/Medicare knowledge is preferred.

Qualifications

• Master’s degree in Social Work required.
• 2 years direct patient care experience required.
• Experience with Medicare/Medicaid CMS regulations preferred.

Responsibilities

• Advocate for patients and families, coordinating care with providers, care teams and community resources.
• Liaise between patients, care team members, SNFs, home health and hospital staff.
• Help patients access community programs and empower them to navigate medical care and services.
