Job summary

Baptist Health Medical Group is seeking a Social Worker (MSW) to join our team. This is a remote position requiring residency in KY or IN. The Ambulatory Social Worker manages case load for patients under value-based contracts, engaging them by phone and creating a personalized plan of care.

Qualifications include MSW from CSWE-accredited program, Kentucky CSW license, reciprocal licensure within 3 months for KY/IN, and CCM preferred; 2+ years in medical social work preferred.

Qualifications

• MSW from a CSWE-accredited program is required.
• Current Kentucky license as a Certified Social Worker (CSW) required.
• Reciprocal licensure in nonresident state within 3 months of hire (KY/IN).
• Reciprocal Tennessee licensure within 3 months of hire for TN residents.
• CCM certification preferred.
• Minimum two years of medical social work experience preferred.

Responsibilities

• Case management of Baptist Health patients under value-based contracts.
• Engage patients via telephonic outreach and develop a personalized plan of care.
• Coordinate community resources and support self-management of care needs.
• Provide emotional support and facilitate Advance Care Planning conversations.
