Baptist Health Medical Group is seeking a remote Ambulatory Social Worker (MSW) to join our team. This role requires residency in Kentucky or Indiana and focuses on case management for patients in value-based contracts.

The Social Worker engages patients by phone, develops personalized care plans, coordinates community resources, and supports emotional and financial outcomes. Collaboration with external providers ensures continuity of care and patient self-management.

Qualifications

• MSW from a school of social work accredited by the CSWE.
• Current Kentucky license as a Certified Social Worker (CSW) required.
• Reciprocal licensure in nonresident state (IN) required within 3 months of hire.
• Reciprocal licensure in Tennessee required within 3 months of hire.
• Certification in Case Management (CCM) preferred.
• Minimum two years of medical social work experience preferred.

Responsibilities

• Manage case load for Baptist Health patients under value-based contracts.
• Engage patients via telephonic outreach and develop personalized plans of care.
• Coordinate community resources and manage psychosocial and financial outcomes.
• Provide emotional support to patients and families and discuss Advanced Care Planning.
• Coordinate continuity of care with external healthcare organizations and facilities.
• Support patient self-management of medical and psychosocial needs.
