TailorCare, Inc. is seeking a Clinical Navigator (PT) to deliver high-quality virtual care navigation for MSK patients. You will engage members via calls, video visits, and digital touchpoints, guiding them through care plans in a fast-paced, metrics-driven environment.

The role emphasizes clinical expertise, operational excellence, and data-driven decisions. You will support virtual MSK care programs and collaborate with cross-functional teams to expand digital therapy offerings.

Competenze

• Doctor of Physical Therapy (DPT) or Masters in Physical Therapy (MPT).
• Bilingual - English and Spanish.
• Current unrestricted Physical Therapy license in Georgia, Texas, Colorado and/ or Indiana (Compact License preferred).
• Willingness to obtain additional state licenses as TailorCare expands.
• Minimum 3 years in physical therapy, musculoskeletal care, care navigation, telehealth, or related clinical settings.
• Strong communication, motivational interviewing, and patient engagement skills.
• Experience using technology and data to improve workflow and outcomes.
• Ability to work independently with accountability to productivity and quality.

Mansioni

• Serve as the primary navigator for an assigned member population in a contact center environment.
• Conduct comprehensive clinical intake assessments for MSK conditions and goals.
• Deliver navigation via telephonic and video-based interactions.
• Maintain a high volume of member interactions with empathy and clinical soundness.
• Educate members on treatment options and recommendations.
• Follow up to drive adherence and address barriers to care.
• Monitor progress and escalate when needed.
• Collaborate with interdisciplinary teams for seamless care.
