Job summary

Independence Blue Cross is hiring a Care Transition Specialist to support members moving from inpatient care to home or another setting. You will promote continuity of care, reduce readmissions, and coordinate services with an inter-disciplinary team.

This fully remote role requires a Bachelor’s degree and 1–2 years in healthcare, with strong communication and client-facing skills. You will work within the Tri-State Area (DE/NJ/PA) to ensure timely follow-ups.

Qualifications

• Bachelor’s degree required in health sciences, psychology, social work, nursing, public health, or related field.
• Medical Assistant certificate preferred.
• 1–2 years in healthcare in a member-facing role (case management or member outreach).
• Experience with transition of care calls, post-discharge support, and patient engagement.
• Knowledge of managed care principles, discharge planning, and community resources.

Responsibilities

• Member Outreach & Engagement: contact members post-discharge; review instructions; educate on meds, symptoms, follow-ups; connect to resources.
• Care Coordination: schedule follow-ups; refer to home health, DME, community services; collaborate with nurse health coach/social workers.
• Monitoring & Documentation: document outreach outcomes; ensure compliance with organizational, regulatory, and NCQA standards; monitor high-risk members.
