Job summary

Boost Home Healthcare - Joliet is seeking a Medical Social Worker (MSW) to provide intermittent social work services in patients’ residences under supervision. The role emphasizes assessing needs, developing plans, crisis intervention, and collaboration with physicians and the healthcare team.

Qualifications

• Master’s or Doctoral Degree in Social Work accredited by CSWE.
• Two years of Social Work experience in a Home Health setting.

Responsibilities

• Provide quality services by assessing patient/family system needs.
• Develop and implement treatment plans in accordance with departmental and medical center policies.
• Perform a bio-psychological assessment interview with a patient, family, or significant other.
• Provide crisis intervention/treatment to adults, children, and families in emergency situations.
• Participate in the development of the Plan of Care and prepare clinical and progress notes.
• Submit assessments and notes timely.
• Provide a professional interpretation of a patient’s condition and recommend psycho-social interventions.
