Job summary

Boost Home Healthcare – Framingham-Norwood seeks a Medical Social Worker (MSW) to provide social work services to patients at their residences, under supervision, per physician orders and care plans. The role emphasizes compassionate, patient-centered care in home settings, with opportunities for advancement and ongoing training.

Key responsibilities include assessing patient/family needs, developing treatment plans, crisis intervention, and timely documentation.

Qualifications

• Two years of Social Work experience in a Home Health setting.
• Master’s or Doctoral Degree in Social Work from CSWE-accredited program.

Responsibilities

• Provide quality social work services by assessing patient/family system needs.
• Develop and implement treatment plans in accordance with departmental policies.
• Perform bio-psychological assessment interviews with patients and families as per policy and standards.
• Provide crisis intervention/treatment to adults, children, and families in emergencies, including information/referral and physician consultation.
• Participate in development of Plan of Care and prepare clinical/progress notes.
• Submit assessments and notes in a timely manner.
• Provide professional interpretation of a patient’s condition and recommend psycho-social interventions.
