Boost Home Healthcare seeks a Medical Social Worker (MSW) to provide social work services in patients’ homes under physician orders and the plan of care. You’ll perform assessments, develop treatment plans, and engage in crisis intervention as part of a collaborative healthcare team.

Requirements include a Master’s or Doctoral Degree in Social Work (CSWE-accredited) and two years of home health social work experience. Flexible work-from-home options are available.

Aufgaben

• Provide quality services by assessing patient/family system needs.
• Develop and implement treatment plans in accordance with departmental and medical center policies.
• Perform a bio-psychological assessment interview with a patient, family, or significant other in line with policy and standards.
• Provide crisis intervention/treatment to adults, children, and families in emergencies, including assessment, counseling, information/referral, and consultation to physicians and the healthcare team.
• Participate in the development of the Plan of Care and prepare clinical and progress notes.
• Submit assessments and notes timely.
• Provide a professional interpretation of a patient’s condition and recommend appropriate psycho-social intervention and/or treatment plan.
