Job summary

Aptive is seeking a Medical Record Technician (Coder) for a remote role to support outpatient coding for the VA Portland Health Care System. The ideal candidate will perform accurate and timely coding, ensuring compliance with hospital standards.

Must have an active coding credential from AHIMA or AAPC and at least 2 years of outpatient coding experience. Responsibilities include reviewing medical records and coding using ICD-10-CM/PCS and CPT/HCPCS. U.S. citizenship is required.

Qualifications

• Active coding credential from AHIMA (RHIA, RHIT, CCS, or CCS-P) or AAPC (CPC or CPC-H).
• Minimum 2 years of experience in outpatient coding.
• Proficiency with ICD-10-CM, ICD-10-PCS, CPT/HCPCS, DRG assignment, and E&M guidelines.

Responsibilities

• Perform outpatient facility coding including ICD-10-CM, ICD-10-PCS, CPT/HCPCS.
• Access and review health record documentation in VA's VistA/CPRS system.
• Clarify conflicting, ambiguous, or nonspecific record information.
