Job summary

Lee Health in Remote - FL is seeking a Medical Coder to abstract data from medical records into Epic and 3M 360, delivering detailed case summaries. You will identify primary and secondary diagnoses and procedures in line with ICD-10-CM and CPT-4 guidelines and ensure proper APC assignments.

Responsibilities include coding SDS, Observation, ED, Diagnostic and Ancillary records, as well as Surgical and E&M records when required, under government payer procedures and guidelines.

Qualifications

• Education: High School diploma or equivalent required.
• Experience: Minimum of 1 Year of outpatient multidisciplinary coding and/or Provider E&M Level of Service Coding (Professional Fee Only) experience required.
• Certification: Minimum one coding certification required (listed below). Other specialty certifications from AAPC or AHIMA will be considered.

Responsibilities

• Abstracts data from medical records into Epic and 3M 360 to provide a detailed case summary of medical, demographic, and statistical information.
• Identifies primary diagnosis and procedure as well as pertinent secondary diagnoses and procedures per ICD-10-CM and CPT-4 guidelines.
• Follows procedures mandated by government and other payers for completion of coded data including APC assignments.
• Facility Specific: Responsible for coding SDS, Observation, and as needed ED, Diagnostic, and Ancillary records.
