Qualifications

• RN Medical Case Manager experience preferred.
• Current RN licensure in California.
• CCM/CIRS certifications preferred.
• Willingness to travel up to 60% or more.
• Experience with workers' compensation or managed care is helpful.

Responsibilities

• Provide in-person and telephonic medical case management.
• Discuss treatment plans with physicians and families.
• Assess, plan, implement, and evaluate patient progress.
• Coordinate with healthcare providers, employers, and referral sources.
• Travel to assigned locations and conduct home visits as needed.
