Job summary

Qualifications

• RN Medical Case Manager experience ideal; nursing clinical background in orthopedics, neurology, or rehabilitation is acceptable.
• Graduate of accredited school of nursing.
• Current RN licensure in operating state.
• CCM/CIRS certifications preferred.

Responsibilities

• Provides in-person and telephonic Medical Case Management to individuals, involving the patient, physician, other health care providers, the employer, and the referral source.
• Discusses current and alternate treatment plans with the physician and other providers.
• Provides assessment, planning, implementation, and evaluation of patient progress.
• Evaluates patient treatment plan for appropriateness, medical necessity, and cost effectiveness.
• Attends doctors, providers, hospital discharge planning conferences, and related visits.
• Conducts home visits for initial evaluation and arranges durable medical equipment and nursing services.
• Role requires regular local travel up to ~60% of the work week, with potential overnight travel.
