Job summary

Talentify is seeking an MDS Coordinator in Illinois to ensure accurate Medicare/Medicaid case-mix documentation and timely care plan completion. You will oversee the MDS/ PPS process and integrate nursing, dietary, social, restorative, and physician services to optimize reimbursement.

The ideal candidate is a Registered Nurse or Licensed Practical Nurse with long-term care experience and restorative certification within six months of hire.

Qualifications

• Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care experience required.
• Restorative Certification required within six (6) months of hire if not already certified.
• Ability to read, write, speak and understand English.

Responsibilities

• Assesses health status and level of care for new admissions.
• Completes MDS assessments and PPS documentation on schedule.
• Maintains schedule of MDS assessments with reference dates.
• Prepares care plans on admission, quarterly and as needed.
• Schedules and conducts resident care conferences and MDS reviews.
• Oversees restorative nursing measures in care plans.
