Job summary

Avenues at Royal Oaks in Illinois is seeking an MDS Coordinator to ensure accurate Medicare/Medicaid case-mix and care plan documentation for optimal PDPM and RUG categorization.

You will oversee MDS/PPS documentation, care plan integration with nursing, dietary, social recreation, restorative and physician services, and coordinate the restorative nursing program to maximize reimbursement.

Qualifications

• Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care experience required.
• Restorative Certification required within six months of hire if not already certified at time of employment.
• Ability to read, write, speak and understand English.

Responsibilities

• MDS Assessment and documentation: assess health status and level of care for new admissions.
• Ensure timely completion of all MDS assessments including PPS Medicare and quarterly reviews.
• Maintain accurate schedule of MDS assessments with reference dates throughout resident stay.
• Care Planning and Resident Assessment: complete care plans on admission, quarterly and as needed per regulatory timeframes.
• Care Conferences and Interdisciplinary Coordination: schedule and conduct resident care conferences and ensure MDS reviews are completed prior to conference.
• Restorative Nursing Program Coordination: oversee restorative nursing measures reflected on care plans.
