Job summary

Arcadia Care is seeking an MDS Coordinator to ensure accurate Medicare/Medicaid case-mix and care plan documentation for optimal PDPM/RUG categorization. You will oversee the MDS/PPS documentation process, care plans, and restorative nursing coordination, collaborating with nursing, dietary, social recreation, and physicians to assure proper assessment and reimbursement.

Qualifications

• MDS assessments and documentation accuracy.
• Care plans on admission, quarterly and as needed.
• Interdisciplinary coordination for comprehensive resident care.

Responsibilities

• MDS Assessment and Documentation for new admissions.
• Ensure timely completion of MDS assessments including PPS Medicare, quarterly, annual, significant change.
• Maintain accurate MDS assessment schedules with reference dates.
• Complete care plans according to regulatory time frames; ensure thorough documentation.
