Humana Inc. seeks a Manager of Care Management (Behavioral Health) to lead a remote team of nurses and behavioral health professionals in delivering comprehensive care management services.

The role emphasizes data-driven decisions, cross‑department collaboration, and enhancement of member outcomes. The position requires an active SC RN license or equivalent master‑level clinical credential, BSN or higher, 5+ years in behavioral health, and 2+ years in management with care coordination

Qualifications

• RN license in South Carolina
• BSN or Licensed Master Level Clinician in psychology or social work
• 5+ years Behavioral Health experience
• 2+ years management experience
• 2+ years Care Coordination/Case Management experience
• Proficiency in analyzing and interpreting data trends
• Experience with Medicaid/Medicare patient coordination

Responsibilities

• Oversees assessment and evaluation of members' behavioral health needs and guides access to appropriate resources
• Makes decisions on resources and tactical operations for projects
• Works cross-departmentally and conducts area meetings
• Contributes to decisions about process/methodology for Intensive Case Management
• Identifies and resolves barriers to effective care
• Understands departmental and organizational objectives and linkages
• Leads a hybrid/remote team
• Supports improving consumer experiences
