Job summary

Tuesday Health is seeking a Licensed Vocational Nurse (LVN) in Houston, TX to join a value-based palliative care provider group. The LVN will work with nurse practitioners and RNs to deliver coordinated care, perform assessments, and help develop individualized care plans that improve quality of life for patients and families in home-based settings.

Qualifications

Active and unrestricted LPN license in Texas.
Experience in clinical/medical setting preferred.
Experience in multidisciplinary care model desirable.
Strong communication and organizational skills.
Ability to multitask and work under pressure while maintaining patient experience.
Willingness to drive during daytime, nighttime, or inclement weather.

Responsibilities

Administer assessments and screening tools with support of nurse practitioners and RNs.
Prioritize needs based on assessment results and coordinate with the care team.
Be accountable for care plan development within the multidisciplinary team.
Participate in rounds to ensure the member experience is optimal and coordinated.
Learn Tuesday Health electronic systems, tools, and technology to deliver coordinated care.
Build a strong, trusting relationship with every member through listening and empathy.
Serve as the quarterback of the multidisciplinary team transforming care for members with serious illness.
