Job summary
Community Healthcare of Texas is currently recruiting an Inpatient LVN to support the RN in delivering compassionate hospice and palliative care for patients with serious illnesses. You will assist with admissions, collaborate with the interdisciplinary group, document care, provide emotional support to families, and help with discharge planning and crisis management as part of a dedicated hospice team.
Texas LVN license and at least 1 year of nursing experience are required; hospice/home health
Qualifications
- Licensed Vocational Nurse license for the State of Texas is required.
- 1 year of nursing experience is required.
- Hospice/home health experience is preferred.
- Proficient computer skills with Microsoft Office suite are required.
Responsibilities
- Assist the RN in the overall plan of care for the patient.
- Provide emotional support for patients and families.
- Coordinate with IDG and supervise CNAs in case management.
- Document nursing care and interactions with patient and family.
- Participate in follow-up with bereaved families.
